Form must be signed by the applicant; for applicants under 16, a parent or legal guardian signs on their behalf. Can be submitted online via GovHK or in paper form via consulate or Immigration Department. Form must specify that the purpose is transit and include details of onward destination and travel dates. Original signed form and photo must be mailed or lodged if application is first sent by fax; online applications upload scanned copies.
Confirmed onward transportation booking from Hong Kong to a third country/region, such as an e-ticket or itinerary showing departure within the permitted transit period.
Must clearly show applicant’s name, flight or vessel details, and confirmed onward destination; open tickets are generally not accepted.

Proof of valid visa or entry permission for the final destination after transiting Hong Kong, if required by that destination.
